600000 Work Type: 48 to 63 Hours Pay Period Work Schedule: MIXED HRS - Varied length shfts-Day-Eve-Nt Exempt from Overtime: Exempt: Yes What You'll Do Summary: The Wound Care Specialist Nurse is responsible for achieving standards of care, collaborating with physicians, managers, clinical coordinators and other team members to assess, plan, implement and evaluate individualized patient care in accordance with the current policies and procedures.
Responsible for ongoing quality, productivity and efficiency in Wound Ostomy Services.
Responsibilities: • Leadership: Provide coordination for wound/ostomy/continence program, services, resources and standards.
Assure completion of required credentialing/skills validation and competency testing.
Assure national WOCN standards of care are implemented.
Demonstrate advanced practice skills with expertise in clinical decision making and independently incorporating concepts of standards in wound/ostomy/continence.
Attend training and seminars to maintain clinical expertise.
Serve as a resource for nurses, physicians and other health care professionals.
Support Shared Governance Model.
• Develop Staff: Delegate management of processes to staff.
Facilitate decision=making within work environment.
Develop and provide formal and informal education programs to assist nursing staff to meet performance expectations and standards of practice.
• Quality Improvement: Assess, plan, implement and evaluate patient plans of care and on the best science, best service and best cost.
Improve the outcomes of patients with wounds, ostomy and special skin care needs.
Identify opportunities to improve core processes based on input from staff and customers.
• Manage Resources: Monitor bed therapy utilization, wound vac utilization, ostomy products, wound care supplies, clinic supplies, etc.
Identify opportunities to enhance efficiency in bed therapy program.
Participate in decision making regarding contract negotiation, capital expenditures and supplies related to bed therapy and wound/ostomy/continence care.
Design, implement, and evaluate cast effective quality and regulatory initiatives.
• Patient Care & Education Counseling: Provide education to the patient/significant others, health care providers and community at large related to disease management, prevention and episodic care.
Document patient understanding/ outcomes of education provided.
Provide feedback on effectiveness of the patient education.
Monitor outcomes of patient/health education.
Provide direct patient care for patients with complex and/or special needs related to wound/ostomy/continence.
• Promote Health & Wellness: Provide overall coordination for wound/ostomy/continence education, information and training.
Implement the education process to meet identified learning needs.
Design, maintain and evaluate education programs/resources to facilitate management of wound/ostomy patient populations.
Participate in community activities to promote a health and wellness focus.
Provide expert content knowledge in field of wound/ostomy/continence programs.
Coordinate availability of current educational resources.
Serve as link to community resources.
• Consultation: Participate in interdisciplinary planning of care.
Works with strategic sourcing managers to determine appropriate wound and ostomy supplies with contracting limitations.
Support staff in the management of complex patient teaching/care for wound/ostomy/continence patients.
• Physical Duties: Walking and flexibility with positioning such as frequent squatting, bending, pushing and pulling is common and required.
Many patients are around 300 lbs.
and a few from 500-600lbs.
• Human Experience: Show courage through creating and sharing innovative ideas to improve the experience for both patients and peers.
Round on patients to create meaningful connections and keep patients informed of visit details (delays/wait times).
Model the experience principles through consistently engaging in Always Event behaviors and viewing feedback through the patient lens.
Recognize and value the unique differences and similarities in both our team members and patients to create an inclusive environment where diversity is celebrated.
Explain all processes to patients in plain language and utilize teach back to ensure understanding.
Know and model the mission, vision and values, and how they relate to role-specific responsibilities.

Position Requirements Credentials: Essential: * Registered Nurse * Certified Wound Care Nurse Education: Essential: * Bachelors Degree Other information: Education: 4 Year / Bachelors Degree - BS or BSN required.
Experience: Required: Three years in clinical nursing.
Licensure/Certification: Required: Current RN licensure in appropriate state.
Wound/Ostomy Certification is required.
Completion of - Wound/Ostomy Course provided by accredited program.
This must be completed within 2 years of hire and certification should be achieved within within 6 months to 1 year after program is complete Demonstrates standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence.
Performs other duties as assigned.
Individual will possess commensurate combination of education, experience and qualifications.
